Contest Rules: This contest is open to WATD listeners and will begin Wednesday, January 8th, 2020 at 6:00 am and end at 5:00 p.m. on Friday, March 13, 2020. No payment necessary to enter. Winner will be chosen at random during remote broadcast at Healthtrax the week of March 16, 2020. Entrant need not be present to win prize. The Prize: a one, one-year Adult individual membership to Healthtrax located at 20 East Street in Hanover, MA, valid for new members only and is non-transferable. Membership cannot be redeemed for cash and cannot be combined with other Healthtrax or WATD promotions and offers. One-year membership is an individual membership, which must be activated before July 1, 2020. Must be 18 years or older to enter. WATD and Healthtrax not responsible for entries misdirected, lost or not received by WATD. One entry per household and one entry per email address – duplicate email addresses will not be considered. Decisions of station management, with regards to this promotion, are final. Healthtrax and WATD employees and their families, as well as employees of other radio stations are not eligible to enter or win. All entrants must provide valid email address and phone number to be contacted if they are chosen the winner. All winners must present valid ID to register at Healthtrax and complete a Healthtrax membership agreement. WATD reserves the right to change promotion, dates, and prize or to terminate promotion at any time. WATD reserves the right to use winner name in promotion of contest both on air and online. Winner of one-year membership to Healthtrax will be required to make payments of all federal, state and local taxes for winning prize valued at $600 or more. Winner may be required to complete and submit to the IRS the form W-9 with winner’s Social Security number for a prize valued at $600 or more. One-year Adult individual Healthtrax membership in Hanover, MA valued at $780.
